Obstructive sleep apnea (OSA) in children with Down Syndrome (DS) is a common problem that is usually treated by adenotonsillectomy (AT), but there is a high rate of failure of AT in this population. We hypothesize that a more personalized surgery based on endoscopic evaluation of the airway during sleep can be more effective in treating OSA. This randomized controlled trial of sleep endoscopy-directed surgery vs AT in children with DS has the potential to change the treatment paradigm in children with DS to a more personalized treatment protocol with improved surgical outcomes and less need for secondary treatments.
